To find out more about your coverage, phone your provider before your first session with us.
Be sure to get the name of the person you’re speaking to and ask the following questions:
Does my policy cover out-of-network outpatient psychotherapy for me? For my child?
Is there a limit to the number of visits allowed?
Is a doctor’s referral required?
Is pre-authorization required? What is the name and number of the person to be contacted?
Is there a deductible? What is the deductible amount?
Is it a yearly deductible? Have I met my deductible for this year?
If not, how much do I have left over to meet?
What percentage of my therapy bills will my policy cover?
What is the address of the office where I should send my claims?
To whose attention is the claim to be sent?
Is there a special form I will need to send with the bill from my therapist?
