Questions to Ask Your Insurance Company


To find out more about your coverage, phone your provider before your first session with us.


Be sure to get the name of the person you’re speaking to and ask the following questions:

  1. Does my policy cover out-of-network outpatient psychotherapy for me? For my child?

  2. Is there a limit to the number of visits allowed?

  3. Is a doctor’s referral required?

  4. Is pre-authorization required? What is the name and number of the person to be contacted?

  5. Is there a deductible? What is the deductible amount?

  6. Is it a yearly deductible? Have I met my deductible for this year?

  7. If not, how much do I have left over to meet?

  8. What percentage of my therapy bills will my policy cover?

  9. What is the address of the office where I should send my claims?

  10. To whose attention is the claim to be sent?

  11. Is there a special form I will need to send with the bill from my therapist?


When you contact your insurance company, be prepared with the information you need to share with them in order to determine your benefits with CFCG as your “Out-Of-Network” Provider:

  • The name of the therapist your family will be working with and license or associate number (available on our staff page)

  • The "specialty" is “child and family therapy”

  • The main phone number (408)351-1044 and fax number (available on request) and street address for CFCG

  • The NPI-National Provider Identification- number is available by searching by the name of the therapist at this website: https://www.npinumberlookup.org/.

  • The “EIN” or Employer Identification Number for CFCG is 274829055.

  • The most commonly used CPT or “procedure” codes in our practice include: 90791, 90846, 90834, 90847. Sessions that include telehealth (98968), shortened sessions (90832), longer sessions (codes may vary), and crisis services (codes may vary) are billed differently and may or may not be covered by your insurance.


Please note:

A diagnosis code cannot be provided until an assessment has been completed, including at least one session with the child. Questionnaires may need to be completed by the parents, teacher or others in order to determine a diagnosis code.


CFCG and our staff do not engage in “single case agreements” with insurance companies.


We can provide no guarantee that any services we provide will be covered by any insurance company.
